VisitRevive is an early-stage workflow product for independent physical therapy clinics. This policy explains the limited information collected through this pilot website.

When you apply for the pilot, we collect your name, business email, clinic name, number of locations, current EMR, and the operational challenge you describe. We also receive basic technical information needed to operate and secure the website.

Do not submit patient information

This website and pilot application are not intended to collect protected health information. Do not include patient names, contact information, dates of birth, medical details, or other identifying information in the application form.

How we use information

We use application information to evaluate pilot fit, respond to you, improve VisitRevive, prevent abuse, and maintain the website. We do not sell applicant information or use it for unrelated advertising.

Sharing and service providers

Information may be processed by infrastructure and communication providers that help operate the website. We may also disclose information when required by law or necessary to protect the website, applicants, or others.

Retention and security

We retain pilot applications while evaluating or operating the pilot and for a reasonable period afterward. We use reasonable safeguards, but no online service can guarantee absolute security.

Your choices

You may request access, correction, or deletion of your pilot application by emailing michael.olearypt@gmail.com.

We may update this policy as VisitRevive develops. The effective date above will be updated when material changes are made.
